Anti-Inflammatory Eating: What It Actually Means and Where to Start

"Anti-inflammatory" has become one of the most used phrases in the wellness space, attached to everything from smoothies to supplements to entire dietary protocols. And like many terms that get co-opted by the wellness industry, it has accumulated a lot of noise, exaggeration, and commercial interest around it that can make it genuinely difficult to know what is actually evidence-based and what is marketing.

The underlying science, however, is real and important. Chronic inflammation is now recognised as a central driver of many of the most prevalent and serious diseases of our time, including type 2 diabetes, cardiovascular disease, certain cancers, autoimmune conditions, depression, and neurodegenerative disease.

What Is Inflammation, and Why Does It Matter?

Inflammation is not inherently a problem. Acute inflammation is a healthy and necessary immune response: when you cut your finger, sprain an ankle, or fight off an infection, inflammation is the mechanism your body uses to protect, repair, and heal. It is time-limited, targeted, and essential.

Chronic low-grade inflammation is something different. It is a state of persistent, systemic immune activation that simmers in the background without a specific acute trigger, and over time it contributes to tissue damage, metabolic dysfunction, and disease progression. It does not necessarily cause obvious symptoms, which is part of what makes it insidious.

Chronic inflammation can be driven by many factors including excess body fat (particularly visceral fat around the organs), smoking, chronic stress, poor sleep, physical inactivity, gut dysbiosis, environmental toxins, and diet. These factors interact and compound each other. Diet is one piece of this picture, but it is a meaningful and actionable one.

The Mediterranean and MIND Diet 

The landmark PREDIMED trial, a large randomised controlled trial published in the New England Journal of Medicine involving over 7,000 participants at high cardiovascular risk, found that a Mediterranean diet supplemented with extra virgin olive oil or nuts significantly reduced the incidence of major cardiovascular events compared to a low-fat control diet.

The Mediterranean diet has been shown to significantly reduce certain inflammatory markers and is associated with reduced risk of type 2 diabetes, certain cancers, depression, cognitive decline, and all-cause mortality across large prospective cohort studies.

The MIND diet shares considerable overlap with the Mediterranean pattern while adding specific emphasis on foods with the strongest evidence for neuroprotection.

What both of these dietary patterns have in common is that they are not built around a list of forbidden foods or expensive superfoods. They are characterised by an abundance of diverse whole plant foods, quality fats, and minimally processed ingredients.

The Foods With the Strongest Anti-Inflammatory Evidence

Extra Virgin Olive Oil

Extra virgin olive oil is arguably the single best-evidenced anti-inflammatory food. It is rich in oleocanthal, a polyphenol compound that has been shown to inhibit the same inflammatory enzymes (COX-1 and COX-2) as ibuprofen. 

It also contains oleic acid, a monounsaturated fatty acid that reduces pro-inflammatory signalling, and a broad spectrum of polyphenols with antioxidant activity. Research shows that populations with high extra virgin olive oil consumption have lower levels of inflammatory markers and lower rates of inflammatory disease.

Using extra virgin olive oil as your primary cooking and dressing fat is one of the most straightforward and well-supported anti-inflammatory choices you can make.

Oily Fish and Omega-3 Fatty Acids

EPA and DHA, the long-chain omega-3 fatty acids found in oily fish, are among the most extensively studied anti-inflammatory nutrients. They are incorporated into cell membranes throughout the body and serve as precursors to anti-inflammatory signalling molecules called resolvins and protectins, which actively help resolve inflammatory processes.

Regular oily fish consumption (salmon, mackerel, sardines, herring, anchovies) two to three times per week is associated with reduced risk of cardiovascular disease, rheumatoid arthritis, depression, and cognitive decline.

For those who do not eat fish, algae-based omega-3 supplements provide a bioavailable plant-based source of DHA and EPA.

Vegetables and Fruit

The breadth of evidence linking vegetable and fruit intake to reduced inflammation and disease risk is enormous. These foods contain fibre, which feeds beneficial gut bacteria and supports the production of short-chain fatty acids with anti-inflammatory effects. They also contain a vast array of polyphenols, carotenoids, vitamins, and minerals that modulate inflammatory pathways through multiple mechanisms.

Particular attention has been paid to cruciferous vegetables (broccoli, cauliflower, Brussels sprouts, kale, cabbage), which contain sulforaphane, a compound with potent anti-inflammatory and antioxidant properties. Berries, which are exceptionally rich in anthocyanins, have well-documented anti-inflammatory effects. Leafy greens including spinach, rocket, and watercress are rich in vitamin K, which plays a regulatory role in inflammatory signalling.

Diversity matters enormously here. Different plant foods contain different phytonutrients, and eating a wide variety across the week provides broader anti-inflammatory coverage. 

Legumes

Lentils, chickpeas, black beans, kidney beans, and other legumes are rich in fibre, plant protein, polyphenols, and resistant starch, all of which support gut microbiome health and reduce inflammatory markers. A systematic review published in Nutrients found that regular legume consumption was associated with significantly lower CRP levels. Legumes are also one of the most affordable and accessible anti-inflammatory foods available. 

Nuts and Seeds

Walnuts in particular have a strong evidence base for anti-inflammatory effects, being one of the few plant foods rich in the omega-3 fatty acid ALA (alpha-linolenic acid) as well as polyphenols and vitamin E.

Flaxseeds and chia seeds are also valuable sources of ALA and fibre. A small daily handful of mixed nuts is a simple yet effective dietary habit in anti-inflammatory eating. 

Wholegrains

Refined grains (white bread, white rice, most breakfast cereals, pastries) have had their fibre and micronutrients largely removed, and their rapid effect on blood glucose and insulin can drive pro-inflammatory signalling. Wholegrains including oats, brown rice, quinoa, barley, rye, and wholegrain bread retain their fibre and phytonutrient content and are consistently associated with lower inflammatory markers in large studies.

Herbs and Spices

Turmeric, ginger, cinnamon, rosemary, and garlic have all demonstrated anti-inflammatory effects in laboratory and clinical research. Curcumin, the active compound in turmeric, has been particularly well studied. 

The caveat with turmeric is that curcumin is poorly absorbed from food alone, and meaningful effects in clinical trials have generally used supplements rather than culinary quantities. That said, using turmeric, ginger, garlic, and other herbs and spices liberally in cooking contributes to the cumulative anti-inflammatory effect of the overall dietary pattern and adds considerable variety and enjoyment to food.

What Drives Inflammation Through Diet

Dietary patterns high in refined carbohydrates, industrially processed seed oils, sodium, and additives, while being low in fibre and micronutrients tend to disrupt the gut microbiome, which is increasingly understood to be a central mediator of systemic inflammation.

Refined sugar and high-fructose corn syrup drive inflammation through multiple pathways including advanced glycation end products (AGEs), increased gut permeability, and dysregulation of insulin signalling. Trans fats, though now largely removed from UK food products following regulatory action, remain present in some processed foods and are among the most pro-inflammatory dietary components identified in research.

Alcohol at higher intake levels is also pro-inflammatory, increasing intestinal permeability and driving systemic immune activation.

A Word on the Term "Superfood"

It is worth being clear that no single food will meaningfully shift your inflammatory status on its own. The concept of the superfood, while useful as a marketing shorthand, creates a distorted picture of how nutrition actually works in the body. It is the overall dietary pattern, sustained over time, that determines your inflammatory baseline, not whether you add turmeric to your morning smoothie while the rest of your diet remains unchanged.

This is actually good news, because it means you do not need to find or afford any specific ingredient. You need to build a consistent pattern of eating that is rich in variety, whole foods, and plants. That is accessible, flexible, and sustainable.

What This Looks Like in Practice

An anti-inflammatory dietary pattern in practice looks like building most meals around vegetables, with quality protein from fish, legumes, eggs, poultry, or meat, cooked in or dressed with extra virgin olive oil. It involves eating oily fish regularly, snacking on nuts and fruit, choosing wholegrain versions of staple carbohydrates often, eating legumes several times a week, and cooking with herbs and spices generously.

It’s a dietary pattern that is enjoyable, varied, affordable, and sustainable, not one built around exclusion, anxiety, or expensive powders.

Chronic inflammation is a real and significant driver of long-term disease risk, and diet can be a powerful tool for managing it. This is not about perfection or purity. It is about the general direction of your eating, consistently, over time.
